At Kids Smile Center, we understand that your time is important. That is why we provide new patient registration forms online that can be printed in the comfort of your home and filled out prior to your appointment. This will save time for yourself and our staff when you come to our office. Once these forms are filled out, you can either:

  1. scan and email to info@kidssmilecenterplano.com

  2. fax to (972) 403-1324

  3. or bring to us during your visit to Kids Smile Center

IMPORTANT: Please note that each patient will need to have all forms filled out individually. All patients under the age of 18 years of age must be accompanied by a parent or guardian.
